Subject's release form

Instructions

Please FOLLOW THE INSTRUCTIONS BELOW EXACTLY.

To give Monash the right to use a photo in which you appear:

  1. Make sure the photographer has supplied you with the photo in question.
  2. You must use your own personal email account to send us your consent.
  3. Copy and paste the release form below into an email.
  4. Attach a copy of the photo for which you are providing your consent.
  5. Use the the following subject text: Portal login pic - subject's release for photos by [ put the photographer's surname in here ... ]
  6. Send your entry to: portal-competitions@its.monash.edu.au

Before you send us your consent make sure you have:

  • used the correct form (this one is for subjects, not photographers)
  • attached the photo you are consenting for us to use
  • have used the subject line EXACTLY AS SPECIFIED IN POINT 4 -  and inlcuded the photographer's surname only.

I am the owner of this email's account. With this message, sent by me, I hereby give to Monash University, its legal representatives, successors, and assigns and all persons or corporations acting with its permission, the absolute and irrevocable right and permission with respect to the reproduction of the attached photographs, in which I am a model, to use, reproduce, copy, publish, communicate electronically and broadcast the same, in whole, in part or in composite, whether altered or enhanced or in conjunction with any other images or text, in still, single, multiple or moving form and using any medium, including but not limited to, print, film, video, CD and the Internet, for any lawful purpose.

I acknowledge that I have no right to inspect or approve the finished product that may be used.

I further release Monash University, its officers, employees and agents and all persons or corporations acting with its permission, from any claims or liability arising from or associated with any form of loss or damage, foreseen or unforeseen, associated with the lawful use of these images unless it can be shown that said reproduction was maliciously caused, produced and published for the sole purpose of subjecting me to conspicuous ridicule, scandal, reproach, scorn and indignity.

I certify that I am not a minor, and am free and able to give this consent.
