The heart is laid open to display the left ventricle and valves. At the apex the left ventricular wall is thinned, fibrotic and bulges outwards. A thrombus 4.0cm in diameter is attached to the endocardium in the ventricular aneurism. At necropsy the coronary arteries showed atherosclerosis. This is an aneurism occurring in the scar of an old myocardial infarct.
