Information for patients

Clinical Quality Registries monitor how effective and safe the care provided to patients is. The ADCQR monitors the management of people with diabetes attending diabetes centres across Australia. It includes people at various stages of diabetes.

The ADCQR will provide important information on the uptake of available diabetes management therapies and technologies and report back on how these are impacting the health outcomes and quality of life of people living with diabetes.

You can choose to opt out of the ADCQR at any time, without incurring any out of pocket expenses, or affecting the level of treatment or care you are provided with. To opt out of the ADCQR, please contact us toll free 1800 491 555 in Australia or email us at adcqr@monash.edu.

If you are willing to allow your data to be included in the ADCQR, you are not required to do anything. Should you decide to take part but later change your mind, you are free to withdraw at any time.
