Report a privacy incident
Staff and students are encouraged to report any privacy incidents, of which they become aware, direct to the Privacy Officer by email at privacyofficer@monash.edu. When reporting a privacy incident please include the following information where possible:
- the date the incident occurred
- the type of information involved - e.g. personal, sensitive or health information
- the affected cohort - e.g. staff, students, other
- the number of individuals affected
- the information involved - e.g. name, ID number, etc
- brief outline of the incident
A privacy incident means a suspected or potential breach of the information privacy principles or the health privacy principles, including:
- the use or disclosure of personal or health information for a purpose that is not authorised by the individual or by law; or
- the loss, unauthorised access, of modification of personal or health information.
