How to make a referral

Opening hours:
Tuesdays, Wednesdays, and Thursdays (excluding Victorian and National public holidays)
9.00am - 5.00pm
Please ensure you include the following information in your enquiry:
- Your child’s name
- Your child’s date of birth
- Your name
- Your email and phone number
- Why you are seeking an assessment
We'll aim to contact you within 5 business days after receiving your enquiry
What happens next
If your child or teenager is eligible for an assessment, you will be sent an intake questionnaire to complete online.
The intake questionnaire will then be reviewed by our clinic manager to determine if the referral is within the scope of practice of our university clinic.
- If the referral is accepted: You will receive a confirmation email that your child has been placed on our waitlist along with waiting times for appointment.
- If your referral is not accepted: We will contact you to discuss other options.
