Monash Online Forms is powered by frevvo, and hosted within the University IT infrastructure, in compliance with privacy and IT security policies.

The frevvo package is a web-based tool for managing forms and approval workflows online. Forms can be submitted online, and if they require approval, they can be emailed to the relevant person for action.

Who can use Online Forms

Monash Online Forms are for any faculty or department within the University who need forms online or to automate approval workflows.

Forms can be set up for use by Monash staff or students as required by the Digital team.

Digital signatures and fully online

There's no need to print and physically sign Monash Online Forms - users just need to log into their Monash account.

Where a signature is required, users can:

• upload an image of a signature
• draw a signature in the form
• select the signature button to auto-populate details based on a user's Monash account.
