Capturing consumers’ values, needs and preferences in their journey through hospital care
The challenge
Models of healthcare have traditionally been diagnosis focused and not designed to support person-centered care. But older people increasingly want to be active partners in their care and healthcare decisions. So how is a digital healthcare solution addressing that need? Through embedding the consumer voice into systems and processes that supports person-centred care across different settings during the healthcare journey.
Our impact
- Co-designed a digital health intervention that captures a patient's values, needs, and preferences, ensuring seamless, person-centred care across different settings.
- Very positive end user feedback – 75% of people approached on an acute medical ward, and 88% of people approached on an inpatient rehab ward, during a six-week testing phase at Frankston Hospital (Victoria) wanted to capture and communicate their needs and preferences using the co-design intervention.
- A multi-site project is underway involving six health services in Victoria to plan for further upscaling.
