Integration of person-centred care planning in a hospital setting

Objectives

We aim to develop and test technological solutions to identify an older person’s individual healthcare values, needs and preferences at the point of care and integrate these preferences into systems and workflows to guide personalised care planning across the entire care continuum.

Did you know?

Models of healthcare for older people have traditionally been episodic and diagnosis-centred. In an era of increased chronic illness and multi-morbidity, there is a clear need for healthcare to transition towards integrated, person-centred and goal-oriented care.

However, aligning care planning and delivery with consumers’ values, needs and preferences is operationally challenging, particularly across settings and clinical specialties.

About the research

We will develop and test technological solutions to:

  • Identify older consumers’ individual values, needs and preferences at the point of care; and
  • Integrate these into systems and workflows to guide personalised care planning across the care continuum.
