Information for participants

We have co-designed a Digital Health Summary that:

  • Can be used by residents, their families and healthcare staff
  • Provides a summary of important health information
  • Was built with residents, their families, aged care staff, paramedics, GPs and hospital staff.

The aim of the trial is to test if the digital health summary is useful, acceptable and practical.

Testing of the digital health summary to commence in July 2024 at Peninsula Health, Ambulance Victoria and Regis Rosebud with subsequent testing at Regis Frankston and Arcare Balnarring.

Staff, residents and relatives at involved sites have been notified of the project via email, newsletters and/or presentations and given access to resources including training videos and information on how to access the digital health summary.

If you do not want a digital health summary made for you or the resident you care for please call 0428 783 235 or click on the ‘opt out’ link below.

If you are a resident you will need to provide your name, date of birth and the name of the aged care home that you live in. If you are a relative or carer you will need to provide the name, date of birth and address of the resident that you care for as well as your name. This information is required for verification purposes.
