Developing a Continuity of Care Model to Support People with Chronic Disease in Indonesia

Evi Ningrum is a PhD student from Indonesia with a professional background in nursing, currently undertaking her doctoral research at the Rehabilitation, Ageing and Independent Living (RAIL) Research Centre within the School of Primary and Allied Health Care at Monash University. Her doctoral research focuses on Developing a Continuity of Care Model to Support People with Chronic Disease in Indonesia, a project that reflects her dedication to strengthening health systems and enhancing the quality of care in low- and middle-income country settings. Guided by her primary supervisor, Professor Keith Hill, and external supervisor, Associate Professor Sze-Ee Soh from the University of Melbourne, Evi’s research combines clinical insight with health systems analysis to address existing gaps in chronic disease management across Indonesia’s primary, secondary, and tertiary levels of care.

Her doctoral thesis comprises a series of interconnected studies that together inform the development of a continuity of care (CoC) model for Indonesia. Using a mixed-method approach, the research examines how nurses and individuals living with chronic disease perceive and experience continuity of care, yielding nuanced insights into their roles and interactions within the health system. These findings inform a Delphi process to develop consensus-based recommendations, followed by a feasibility assessment of the proposed model’s practicality and acceptability. Collectively, this work provides a strong, contextually grounded basis for strengthening continuity of care for people with chronic disease in Indonesia. Her PhD project reflects her long-term vision to influence policy, improve healthcare integration, and contribute to transformative change for people living with chronic disease in Indonesia.
