Testing positive or being a household contact

If you have tested positive or you’re a household contact of someone who has tested positive for COVID-19, you need to complete the COVID-19 Self-Reporting Form.

Registering your vaccination status

To attend a Monash campus in Australia you need to register your vaccination status, even if you’re medically exempt, using the Monash COVID-19 Vaccination Status Form.
