ENGAGE Project

Previous research examining medication management across transitions of care has tended to focus on health professional collaborative models, such as pharmacist-general practitioner models. However, these health professional models only represent a partial solution. New strategies are needed to support older people to be engaged in making medication decisions across transitions of care, especially those with complex care needs; otherwise problems will continue, leading to patient harm and avoidable hospital admission. This project involves working in the complex space of transitions of care to inform the development of strategies that comprise what issues are really important for older people and their families. The project contributes to enabling better models of health care and services aimed at improving outcomes, reducing disparities for older people, who are a major disadvantaged and vulnerable group, and increasing efficiencies of care.
