Medication safety involves examining the environmental and sociocultural aspects influencing medication communication, the characteristics of medication communication encounters between patients, families and health professionals of different disciplines, including the power and hierarchical issues that can influence communication, and the outcomes of medication communication. These outcomes include patient participation in medication decision-making, interdisciplinary collaboration, and the prevalence and characteristics of medication errors.

New strategies are needed to support older people to be engaged in making medication decisions across transitions of care, especially those with complex care needs; otherwise problems will continue, leading to patient harm and avoidable hospital admission.
This application is for a capacity building grant through the Australian Association of Gerontology, for early career researchers. The focus for our application is aged care, medication administration, communication/engagement processes, and consumer input into care.
