Use this guide to explain to practices the best way to use the REACH resources. You can personalise the information with your PHN information.

In addition to the resources for practitioners and patients, we have developed a short video for PHN staff to explain what the REACH program is and how it can be implemented. This could be distributed to staff by the manager responsible for the REACH rollout in your PHN.
