Australian HIV Observational Database (AHOD)

The Australian HIV Observational Database (AHOD) is an observational cohort study of more than 4,000 HIV positive patients in Australia under routine clinical care. Approximately 30 HIV clinics throughout Australia and New Zealand participate in AHOD. AHOD data are aggregated by researchers at the Kirby Institute on behalf of collaborating sites.

AHOD has described the changing spectrum of causes of death, and patterns of long term immunological and virological response among treated HIV-positive patients. Over the past 15 years, AHOD has demonstrated incremental improvements in virological response for patients initiated on combination ART. Most deaths are now non-AIDS related, and data from AHOD has underscored the fact that if CD4+ counts are at a high level, life expectancy approaches that of the general population.

Melbourne HIV Cohort Study

Objectives

  1. Establish and follow a cohort of 100 HIV treatment naïve patients with 100 HIV negative controls, in order perform and facilitate a wide range of high quality research projects of relevance to people living with HIV.
  2. To create a partnership and infrastructure that encourages high quality collaborative HIV basic, clinical and behavioural science research.

Factors associated with the failure of antiretroviral therapy in HIV-infected adults (PAART)

Primary study objectives

To identify patient, treatment, and socio-economic characteristics predicting antiretroviral therapy (ART) failure over 24 months in HIV-infected patients receiving stable ART.

Australian Positive & Peers Longevity Evaluation Study (APPLES)

Primary study objectives

A comparison of HIV positive (HIV+) and HIV negative (HIV-) participants for a number of key demographic characteristics at baseline. Specifically to perform a cross-sectional analysis of surrogate markers of age related comorbidities, including:

  • Rates of dyslipidemia, hypertension, smoking, vitamin D deficiency
  • Elevated levels of biomarkers of inflammation such as D-dimer, hsCRP, cystatin C, and IL-6.
  • Further, to estimate and compare the 5 and 10 year predicted risk of CVD and bone disease (osteoporosis and hip fracture) using conventional risk calculators such as the Framingham and FRAX scores
  • Assess feasibility of conducting a large ongoing cohort study

HIV Immunity and Cancer Biobanking Study (HI-C)

Primary objective

Establish a prospectively characterised clinical cohort and biorepository of patients with HIV and malignancy, to serve as the basis for clinical and translational studies in oncogenesis and HIV pathogenesis.

HealthMap

HealthMap is a model of care for people with HIV (PWHIV) that includes use of an interactive shared health record and self-management support. The aims of the HealthMap trial are to evaluate engagement of people with HIV (PWHIV) and healthcare providers with the model, and its effectiveness for reducing coronary heart disease risk, enhancing self-management, and improving mental health and quality of life of PWHIV. The study is a two-arm cluster randomised trial involving HIV clinical sites in several states in Australia.

Safety and Efficacy of Bictegravir/Emtricitabine/Tenofovir Alafenamide Versus Dolutegravir + Emtricitabine/Tenofovir Alafenamide in HIV-1 Infected, Antiretroviral Treatment-Naive Adults (Gilead 1490)

ClinicalTrials.gov Identifier: NCT02607956

Summary

This Phase 3 study will evaluate the efficacy of a fixed dose combination (FDC) containing bictegravir/emtricitabine/tenofovir alafenamide (B/F/TAF) versus dolutegravir (DTG) + a FDC containing emtricitabine/tenofovir alafenamide (F/TAF) in HIV-1 infected, antiretroviral treatment-naive adults at Week 48.

ADVICE: Attenuation of D-dimer using Vorapaxar to target Inflammatory and Coagulation Endpoints

ClinicalTrials.gov Identifier: NCT02394730

Summary

ADVICE is a randomised, international, double-blind, placebo-controlled trial. The purpose of the ADVICE study is to compare the safety and efficacy of vorapaxar in reducing d-dimer expression and markers of cellular immune activation over a period of 12 weeks among people with HIV infection who are successfully treated with combination antiretroviral therapy containing an HIV integrase inhibitor. A secondary objective of the study will be to demonstrate that following cessation of vorapaxar in patients with well controlled HIV replication there will be an increase in the levels of d-dimer over a 6 week period. 60 participants from 4 clinical sites in Australia and the USA will be recruited and followed for a minimum of 18 weeks.
