      We encourage you to personalize the poster with all the names of the GPs and nurses in your practice. This helps patients to know that you are all willing to talk about alcohol.

    • Print this questionnaire to give to patients while waiting for their appointment to assess their overall health (smoking, nutrition, physical activity and alcohol intake), as well as their interest in smoking cessation and alcohol intake reduction approaches. Patients can take completed forms in to discuss with their GP/nurse.

    • Use this visual chart during a consultation as a prompt for the steps to delivering an alcohol brief intervention. It covers asking about alcohol intake, advising patients about the risks from their alcohol use, assessing readiness to change and motivational interviewing tips, and referral options. The brochure presents the health harms of alcohol use, benefits to be gained from reducing alcohol intake, practical strategies for reducing intake, a standard drinks guide, national recommendations on alcohol intake (NHMRC) and where to go for more help. The brochures are appropriate for low-literacy and CALD groups.
