Report an incident or hazard in SARAH

All health, safety and wellbeing hazards, incidents and near misses should be reported through Monash’s online safety system, SARAH.

Through SARAH, all reports involving staff, students, visitors and contractors are recorded, investigated and actioned in line with Victorian OHS legislation and Monash University’s Managing HSW Hazards and Incidents Procedure.

What should be reported?

Any work-related hazards or incidents that occur at Monash University campuses or locations must be reported, regardless of whether they involve staff, students, contractors or visitors.

The report is a record of what happened, how it was identified and any actions taken to reduce risk and prevent it happening again.

Understanding hazards and incidents

  • Hazard: A hazard is anything that could pose a risk to a person’s health, safety and wellbeing (including unsafe work practices), or the work environment (e.g. air, water or land).
  • Incident: An incident is an occurrence arising out of, or in the course of, work that could or does result in injury or ill health. An incident could be:
    • a near miss, which could have led to injury or illness but didn’t
    • an injury or illness (something that has an adverse effect on a person’s physical, mental or cognitive condition).

Hazards and incidents could also include:

  • allegations of unacceptable behaviour (e.g. offensive language or comments)
  • environmental incidents that may be hazardous to a person’s health, safety or wellbeing (e.g. someone disposing of hazardous chemicals into a normal sink).

All reports need to be made through SARAH. Paper-based hazard and incident report forms are no longer accepted.

For supervisors and managers

If you’re a supervisor or manager, you’ll need to ensure hazards and incidents reported by staff and students under your direction are resolved. This involves managing investigations, assigning actions and monitoring progress.

You can transfer reports to another person and delegate actions so others can manage hazards and incidents on your behalf.
