The Lancet publishes ICHOM Major Injury Outcome Set

A globally endorsed International Consortium for Health Outcomes Measurement (ICHOM) Major Injury Set, chaired by Monash University, has been published in The Lancet eClinical Medicine. This framework establishes a new global standard for measuring outcomes in major injury care, moving far beyond mortality to capture a holistic picture of health and wellbeing.

The ICHOM Major Injury Set was developed by an international team of 28 major injury experts and individuals with lived experience from 11 countries, and chaired by Professor Belinda Gabbe, from Monash University’s School of Public Health and Preventive Medicine, with research led by Research Fellows Colonel Henk van der Wal (Ministry of Defence; Erasmus MC, Netherlands) and Winny Collot d'Escury (Leiden University, Netherlands). Professor Gabbe said the framework defines 26 essential and patient-important outcomes that span patient-reported health status, psychological wellbeing, functioning, and clinical outcomes.

"The development of the Major Injuries Set represents a critical step forward in improving trauma care and ensuring that measurement of outcomes is focused on what is most important to the millions of people who experience serious injury each year,” Professor Gabbe said. The multi-national team involving clinicians, researchers and people with lived experience worked tirelessly to deliver this unique and valuable resource."

The Major Injury Set was developed through a rigorous process between May 2024 and April 2025. This included a comprehensive literature review, eight expert meetings to refine the scope and methodology, and a three-round Delphi survey that achieved at least 80 per cent consensus on every outcome. To ensure global applicability, validation surveys were conducted with 121 patients and 70 healthcare professionals with experience in both civilian major injury and military settings. The final framework also defines case-mix factors and follow-up intervals, enabling consistent measurement worldwide.

Until now, major injury treatment outcomes have been fragmented, typically focused on survival or short-term measures of recovery. By establishing this comprehensive global standard, the team have created a foundation for consistent evaluation of patient health, allowing systems to identify care gaps, compare performance internationally, and generate evidence that can drive policy, funding, and clinical improvement. It will serve as a catalyst for advancing clinical guidelines and routine care worldwide. The next step is for stakeholders to start using these measures to move the needle forward.
