Must be completed for activities such as Monash organised events, off-campus activities and travel.
The Travel Risk and Safety office is here to create a culture of proactive travel risk management to support the safety of staff and students across Monash before, during and after University-related travel.

For the student to complete and sign, preferably on their first day of a placement.
Must be submitted for approval to the Diving Safety Officer (DSO) prior to conducting diving activities.
Used to assess the risks associated with a particular dive site, or a group of dive sites with similar attributes and located within a reasonable distance.
Used to evaluate essential and optional skills of a diver. This form also includes snorkelling skill evaluation.
Before any staff/student can dive with Monash they must complete this form and forward it to the Diving Safety Officer for approval.
