New electronic OHS hazard and incident reporting system

Monash has recently launched an online Occupational Health and Safety (OHS) hazard and incident reporting system, the first module in the Safety And Risk Analysis Hub or S.A.R.A.H.

Driven by the Occupational Health and Safety unit within Facilities and Services, this innovation offers staff and students a user-friendly way to report OHS related hazards, incidents, and allegations of unacceptable behaviour. 

The intuitive system replaces the familiar paper based process and provides supervisors, managers and safety officers with ‘live’ accurate information on reported OHS events, while reducing the time and effort required to investigate and action reported hazards and incidents. 

Other benefits include the ability to attach images and documents to reports; automated email notifications to stakeholders upon an incident or hazard being reported; provision of real-time information and comprehensive reporting tools; and escalation of responsibility if required actions are not completed within prescribed time frames.

What should be reported?

Staff and Students are able, and encouraged, to access the system via the my.monash portal and OH&S unit website.

Incidents which should be reported are:  

  • hazards that could pose a risk to:

    • a person’s health or safety, including instances of unsafe work practices

    • the environment (e.g. air, water, land)
  • incidents such as University- related injuries, illnesses and near misses to staff, students, contractors and visitors

  • allegations of unacceptable behaviour
