Permission to release personal information

The privacy laws arise from Victorian legislation.  Consequently, the Monash University Privacy Policy applies to personal information that a person provides to Australian campuses of Monash University. 

As part of your sponsorship requirement, your sponsor may request Monash University to disclose your personal information to them.  Examples of personal information include your student ID, email address, tuition fees amount, academic progression details and others. In order to meet this requirement when administering your sponsorship, the University requires your consent.  If you agree to the release of your personal information to your sponsor, please print and complete a Disclosure Consent form.
