Incidents on placement
All incidents and near misses must be reported. This includes anything that makes you feel unsafe or impacts wellbeing, including (but not limited to):
- Sexual harassment
- Being sworn at or verbally abused
- Any behaviour that makes you feel unsafe
1. Public schools
All incidents in Victorian Government schools must be reported via eduSafe.
- How to report: Ask your mentor or OTP (organiser of teacher placements) to help you lodge report through the eduSafe online.
- Cannot access eduSafe online?: If you are unable to submit eduSafe online, request a printed eduSafe form and submit it through the usual school process.
- Notify Monash: Notify the Monash PPC team as soon as possible - Email edu-ppc@monash.edu with a brief summary of what happened (include the date, time, and centre name).
2. Catholic, Independent, or Specialist schools
If you are placed in a Catholic, Independent, or Specialist School, you must:
- Follow school policy: Follow the school’s incident reporting policy and process for lodging an incident (this may be a different reporting system or form to eduSafe).
- Seek support: Ask your Mentor or OTP to support you to identify the correct process and to help you lodge the report.
- Notify staff: If required by the school, also report the incident to the appropriate staff member (e.g., supervising teacher, Wellbeing Leader, HR, Principal/Delegate) according to the school’s policy.
- Notify OTP: If you are unsure what applies, or you feel uncomfortable reporting alone, tell your OTP immediately so they can guide you and escalate support as needed.
- Notify Monash: Notify the Monash PPC team as soon as possible - Email edu-ppc@monash.edu with a brief summary of what happened (include the date, time, and centre name).
3. Early Childhood Centres
If you’re on placement at an Early Childhood Centre and an incident occurs, you must:
- Follow school policy: Follow the centre’s incident procedure immediately
- Notify staff: Report the incident to the appropriate staff member and lodge the incident with the Centre Manager/Director as per the centre’s policy
- Notify Monash: Notify the Monash PPC team as soon as possible - Email edu-ppc@monash.edu with a brief summary of what happened (include the date, time, and centre name)
