About the STEP UP website
Welcome to STEP UP - Students Transitioning to Expert Practitioners during Undergraduate Placements.
This web-based module has been designed to help undergraduate students understand professional roles and develop key counselling skills to assist patient behavioural change in chronic disease management.
Developing your expertise will involve periods of learning, planning, practising and reflecting. When working through this module, you will undertake these activities which will assist this learning process:
- Reviewing ‘real-life' case studies filmed in the Pharmville community pharmacy
- Identifying best practice for managing different conditions and the key roles for community pharmacists
- Planning how to use these skills in practice
- Reflecting on the counselling you undertake in practice to understand what has worked well and what could be improved.
Hypertension and tobacco use are the conditions of interest in these case studies. These two conditions were chosen for their high prevalence and significant contribution the national burden of disease. While your knowledge of these conditions will improve with the module, the principles of patient management - applicable to most chronic conditions - are the main emphasis.
By the end of this module you should be able to identify the general key roles for pharmacists in facilitating patient behavioural change and to apply them in practice for hypertension management and smoking cessation.
