Arrange follow-up (e.g. to monitor blood pressure or patient adherence to recommendations) and referral to GPs, specialist services, social support groups (e.g. exercise groups), phone information/counselling services (e.g. QUIT for smoking cessation).
The table demonstrates how in the case studies Laura arranged various activities. Instead of being confrontational, Laura at times gives the patient flexibility of when to do things by inviting the patients to initiate the actions (e.g. inviting Tracey to return).
