Advise patients by providing brief information, written where possible.

Assist patients by providing self-management support to help them better manage their condition, or by providing/recommending pharmacotherapies where possible (eg nicotine replacement therapy to reduce cravings).

Discusses with Tracey facts around weight gain and smoking cessation.
Advises Tracey on benefits of smoking cessation, and benefits of a quit plan.
Provides Tracey more educational material.
