Medication Management Review - PDP1001

Develop the required knowledge, skills and attitudes to perform Medication Management Reviews (MMR).

The Monash University Medication Management Review Training Program (MUMTP) is designed to train pharmacists to develop the necessary knowledge, skills, and attitudes required to become credentialed to perform Medication Management Reviews (MMRs) in accordance with the new Australian Pharmacy Council (APC) Performance Outcomes.

Monash University Interns that have completed the IFP Aged Care Training Program within a year of enrolment are eligible for a discounted price of A$850 to complete the remaining components of this program

What you will learn

In this course, you will:

  • Practise legally and ethically by complying with and maintaining contemporary knowledge of the legal, professional and ethical frameworks that relate to pharmacy practice within the aged-care setting, including legislation, pharmacy practice standards, codes and guidelines
  • Discuss and apply clinical and contemporary aged-care related pharmacy knowledge to deliver person-centred care
  • Implement and deliver Medication Management Reviews according to the needs of consumers, taking account of the consumer’s rights and expectations, thus delivering ‘person-centred’ care
  • Take an accurate best possible medication history in the context of performing an MMR
  • Conduct comprehensive MMRs to identify potential or actual medicine-related problems
  • Demonstrate problem solving skills, therapeutic reasoning, application of professional judgement and individual and shared decision-making to develop person-centred medication management plans
  • Develop strategies to monitor and review a person’s response to a medication management plan following an MMR
  • Assess minor conditions and provide management recommendations, within scope of practice, including pharmacological, non-pharmacological and referral options
  • Apply evidence-based principles and skills in research, review and analysis to the development of MMRs and respond to medicines enquires
  • Effectively and appropriately communicate medication and health-related information with a socially and culturally diverse range of people, including Aboriginal and Torres Strait Islander people
  • Effectively communicate and document recommendations to other healthcare professionals using standardised tools to optimise patient care
  • Demonstrate interprofessional collaboration
  • Apply the principles of governance to monitor and improve their service as an MMR pharmacist and to promote and contribute to quality use of medicines for persons under their care

Program structure

The MUMTP consists of three steps, following an active learning approach. Recognition of prior learning (RPL) may exempt you from completing all steps.

Step 1 - Self-directed Online Learning:

Engage in interactive online modules developed by leading experts to consolidate your understanding of the MMR process and the management of older patients.

Steps 2a and 2b - Simulated Workshops and Assessments:

Practice oral communication, interprofessional collaboration, and problem-solving skills in simulated MMR-style workshops (2a) and assessments (2b).

Step 3 - Experiential Learning:

Demonstrate competency in core MMR processes through workplace-based assessments supervised by an experienced MMR pharmacist.

This program is accredited by the Australian Pharmacy Council.

Accelerate your qualification

Learners who have completed the MUMTP micro-credential may be eligible to apply for 6-credit points of credit towards an award course, including P4005- Graduate Certificate of Advanced Pharmacy Practice (subject to credit limits).

You will learn from a range of facilitators including MMR pharmacist with expertise in aged care.
